One of the locations Diablo 4 players can discover is the Malignant Burrow, which is a special underground area and is home to the Echo of Varshan boss.
Season 2 of Diablo 4 introduced Vampires, a new enemy type which has have descended upon Sanctuary to send its legions of newly-turned followers throughout the land to gather the blood of innocents.
Only you can wield the enemy’s blood magic against them and infuse your own armor to put a stop to their sinister plans.
The latest season of the game introduced various new features. Players can now follow a new questline, defeat new enemies, acquire new items and discover numerous new areas on the map.
One of these locations that you can discover is the Malignant Burrow, which is a special underground location.
After activating this area, it serves as a summoning point for the endgame boss, Echo of Varshan. You should note that the burrows are distinct from the main game world and can be accessed through small holes, or craters in the game.
To find the Malignant Burrow, you have to travel to the northeast of Hawezar, near the Tree of Whispers. That being said, you need to complete all the main missions in Diablo 4 to reach the Malignant Burrow.
When you have completed the quests, you can accept the quest, Whispers of the Dead. To find the Tree of Whispers, you need to head northeast of Hawezar and an icon will appear on your map. The icon looks like a red tree with a white skull at the top.
Whisper of the Dead activities
Players should regard Whispers of the Dead tasks as akin to bounties. Every time you complete a task, you will receive a number of Grim Favors or points.
The following table lists the Whisper of the Dead tasks that you can complete and the number of Grim Favors that you can earn:

How to activate the altar in the Malignant Burrow
When you have gained access to the Malignant Burrow, you need to acquire certain items to activate its altar. If you are playing on World Tier 3, you need to procure a Gurgling Head, a Blackened Femur and a Trembling hand.
World Tier 4 players need the same items as World Tier 3 players, in addition to a Malignant Heart.
To acquire these items, you have to open Whisper Caches by offering the Tree of Whispers 10 Grim Favors. Additionally, you can obtain these items by vanquishing Grotesque Debtors.
Keep in mind that you can also craft the summoning items by using specific materials listed below:

Echo of Varshan
Once you have all the summoning items, you can activate the altar in the Malignant Burrow to summon the Echo of Varshan. We recommend that your character is at least level 55 if you are fighting the boss on World Tier 3.
If you cannot reach the 70 percent Poison Resistance cap, you can use the Potent Elixir of Shadow Resistance to overcap your Resistance.
In order to increase your survivability during the fight, we recommend that you remain behind the boss to avoid its frontal attacks.
During the fight, Varshan will spawn triangular void zones that travel away from and back to the boss. You should avoid these zones to survive the fight.
At the beginning of the fight, the boss will spawn 2 Elite adds, which you should defeat as soon as possible.
